Cartilage defects and osteoarthritis are acknowledged as a disease of the entire osteocartilaginous unit, rather than a disorder limited to the surface cartilage alone. Whilst marrow stimulating procedures are generally considered as a first line of treatment for full-thickness osteochondral lesions, there are concerns over the durability of the repair tissue and clinical outcome, especially in larger defects.
